Abstract
This paper is an evaluation of the Farm Record and Monitoring Form designed and used by the Office of the Provincial Agriculturist of the Province of Cotabato. The evaluation was aimed at identifying the economic uses and importance of the data and information that the forms would provide. The researchers recommend the adoption of such forms by the Department of Agriculture
